Default continuous failure skull?

Had a lock-in at the local arcade and played ITG2 for a good 2 hours on freeplay. Towards the end, I decided to jult play as many hard songs as possible. I played Vertex^2 and found about the whole "round failed^2" thing and I got a big kick out of that.

A guy that I played ITG with said he already new about it. I asked him if there was anything else along the same lines and he replied "what, you've never seen the skull?" I ftw'ed there for a second, and he just kinda grinned and walked away.

After chasing him down, he told me that if you play generally-difficult songs (11s and higher, he assumed) on the ITG1 home version (he said he was sure it happened on the home version, but wasn't sure about arcade cabinets) and you fail "every now and then" you will eventually see that a round failed screen shortly thereafter leads to a skull graphic that comes up.

So, is he just pulling my leg? Or is this real?
