You probably have one of three problems.

1) The bpm of the song is incorrect. The newest alpha of StepMania 4.0 has a feature that will correct a song with a constant BPM based on your steps.

2) The bpm of the song is changing. You need to either carefully sync it by hand, or find a program that will let you sync such a song; StepMania doesn't have such a feature.

3) The song is poorly cut, as in the simfile author tried shortening a song and didn't line up the beats correctly when he removed a section in the middle. If the cut was too short, you can fix it by removing a few milliseconds using a wave editor and then resaving the file, but this will hurt the quality. If the cut was too long, you're pretty much screwed. If you really love the song, get a copy of it and recut it yourself.
