At the title screen, enter the codes in these order (each # applying to the button from left to right) Make sure you have a credit in the machine and then perform this combo before it switches to the next screen in the demo. You should hear a noise and an object should appear in the upper left corner (a heart shape?)

This is from the Carnival Website, it appears (correct me if I am mistaken) that some options require e-amuse to unlock but I am uncertain. Here are the codes in order from first to last:

After everything is unlocked, you should get MAX in the upper left corner.

Let me know if these work. I'm not sure if MAX requires e-amuse or not, but I think you should be okay.
