How rigid is your playstyle? At lower difficulties, having a very well defined home position and sticking to it can help you focus on reading/timing/etc, but at higher levels it can kill you.
You need to be able to adapt your playstyle for whatever is going on. I broke myself of this back in 10th style. Forced myself to use a different hand position and drilled Hope of Tomorrow for a few sessions, made some patterns so much easier. Now I'm able to switch back and forth between positions mid-song, to catch different types of patterns.
