Well in the Stapmania3.9+ version I have...
Stops:
1. Make sure you have F-Lock like... ON lol.
2. In the song edit screen (where you can place steps and stuffs) make sure you already have the correct BPM and Beat-Offset punched in.
3. Use F9 to decrease stop time and F10 to increase stop time. (You should see different colored numbers on the left side of the track).
The amount of time that you need to use differs with what the BPM is, and how long you want to have it stopped before it releases into the next set of arrows.
To make stops seem like they're skipping, just double/quadruple the BPM in the area with the stops.
BPM Shifts:
1. F7 and F8 (I'm sure you knew that.)
For the complex ones, they require a little math ( god forbid D: ) and foresight.
The easiest way to see how it's done would be to just peek at some songs in the step editor.
As far as how I'm physically able to handle stops and BPM shifts, I usually do terrible on a sight-read first and then do better on the second play.
