Best moment was when i passed Greenhorn on crazy. that was my 5th time i played PIU.
Worst moment was after passing it, my feet started to hurt like hell. i had to sit down on one of the racing arcade machines.

If you're talking about falling off the pad or slipping off the edge, ^^U...

All the time i put my foot too far out and slide off the corner of the pad and mess up, or i bring my feet too far back in a '13' jump and hit the back of my feet on the bar. Or.. yes, OR... i get gum stuck on the bottom of my shoe and... well you can guess the rest. 
EDIT: Canon D remix is actually easier than the regular IMO.

Best: Don't know, really... passing Monkey Fingers CZ with an A with my soft pad almost facing away from the screen would be my best as it was hard to keep an eye on the screen while playing at an almost 90 degree angle towards the end.
Worst: I want to say it was one time on Turkey March. It was raining that day and I walked to the arcade to play it during that time. I began playing and thought my shoes were dry. Apparently not, as my foot slid off the topright panel and I almost hurt my back as I was holding onto the bar. It scratched the side of my leg. It didn't help matters there was a crowd behind me. They cheered though when I went back on the pad and still A'd the song. >_> I wasn't too thrilled...
