Well I know O2Jam's system because I was a very involved player back in the day but to be honest I don't really remember that problem.

That's because there weren't many charts that actually aggravated the problem. After Blade.

It's something that chart designs had to take into account, not so much players. Even so, anything that was a stream with 16ths that had every other note in a single column (e.g. Bach Alive) could potentially be affected by that since the BAD judge dimension was slightly over a quarter (an 8th in either direction).

You can thank the people who made charts for working around the horrible system.

Fun trivia: There was this awesome bug/issue involving pills and long notes. If you scored a 'bad' on the start of a long note, it deducted a pill, made that bad into a cool, then proceeded to auto-miss the end of the long note anyway. It was introduced in the same patch that took away the dark gray/light gray alternating columns design. That's why some people who had that certain pattern for scoring combo through the staircase pattern in EFHD could do it prior and then couldn't... it had nothing to do with judgement.

Became a common device for anti-spammage pseudo-difficulty in chart design after a while.
