what do i type in these 2 boxes for registration??

there are these 2 boxes that i dont know what to type in

the upper box should be "username" while the lower one is "nickname"

I think it would be better if you just gave me the URL to the site and upload that picture to imageshack.us. I don't feel like browsing through all your pictures. Also, try using common sense: I can only guess it's inside the registration form, so...Are they the first 2 boxes? If so they're probably the login ID and the nickname. If they're hidden they're probably the password. If they're after that they're probably email or something.
