Day Dream and TL100s aren't necessarily the hardest; they CAN be learned just by whoring it out in training mode a whole lot (which I plan to do)... So, if you're some kind of anemic, chicken-armed emo kid...
FUNKY SONIC WORLD would be the hardest.

But under normal circumstances, yeah DD and TL100s are the hardest.

On GF, I'd probably say it'd be one of the Model DD songs just because nobody bothers to properly learn them?

On DDR, FELMO and FaxX

On IIDX, Mei, Nageki, AA, Sasoribi, followed by FaxX, Sense2007, a few other 12s I haven't heard of, thenn OML.
OML is by far the easiest 12 (that i have seriously whored out) outside of like Innocent Walls, even with the bullshit ending I have 50% gauge, as opposed to the other top-tier 12s where I'd be lucky to escape with more than 20%.

It doesn't take that much arm strength to pass FSW, to play it properly you have to learn how to use your fingers/wrists properly. I've manage to get 85% perfect on it just by learning how to relax myself.

I find FAXXO harder then FELMO's chart harder to read (on the screen anyway). FELMO has more stupid jump patterns while FAXXO has longer 400bpm 8th streams.

HDVO (not sure why its not mentioned yet) has 8th streams at 'only' 360bpm but it involves crossovering, so its just a more hax version of PSMO.

Still, Vertex^2 expert > all.
