I alternate.
If I'm going for a PA, I'll usually flatfoot, i.e. hit the pad as hard as possible to make sure that I stay on beat.
If I'm going for speed, I'll hit the forward arrows with my toes, and the backward arrows with my heels to conserve energy (you can't flatfoot FAXX).
Sometimes I'll simply do a combination of both in one song depending on what elements there are.
