If you play on HAZARD and you fail any one song due to hazard, irregardless of how well you do (Example: You got all Perfects for your song before you screw up and got 2 Poors, failing your stage), you will get an E/F grade for that song and hence, you can never get any Extra stages.... Same goes for Encore and Premium stages....
So if you want Extra/Encore/Premium stages easily, DO NOT play on HAZARD!
Note: I am aware of an issue regarding people having an easy way getting Premium/Encore stages by abusing the HAZARD mode..... Simply get 95 to 97 Perfects before intentionally failing your Extra Stage to get an Encore stage. For a Premium Stage, get 98 to 100 Perfects and then fail.... This exploit will make the machine think that 100 Perfects=100% Perfects for that Extra Stage, hence giving the player Premium Stage....
Fortunately, this exploit was fixed later on for E-MUSE ENABLED machines... So, DO NOT try this at any E-MUSE machines.... And I am very sure that this will be fixed for all V3 machines....

Marketplace Rating: 0 | No, poors (which also break the combo) can penalize you in Hazard as well as misses. Basically if you can't full combo the song, you shouldn't try to Hazard it...

Marketplace Rating: 0 | To reinforce, basically in HZD mode you start with two "chances". Anything that makes you break a combo will eliminate one "chance". Once both have been eliminated, you fail the stage.

In GF HZD mode, a wrong note pressed at the same time the correct note hits the bar will count as only ONE miss aka Light Mode. An extra pick immediately preceding the miss of the wrong note will cause another miss, which will fail the song. So, if you only pick normally and pick a wrong note, you will get only ONE miss. If you panic and see the miss and quickly try to pick again when there are NO notes within the judgement area, less the wrong note that has been accounted for, THEN you get another miss.

Say there is 1 note coming, your fingers were in the wrong combination, you get 1 miss. Then you panic and pick another one where there is nothing, you won't get an extra miss from that. | |

I've encountered this alot of times when I was fooling around with HZD, and the Light mode only applies when there is a note within the judgement section, and Light mode gets automatically switched off when there are no notes within the judgement section.
If you think about it, if it's like what you said, then everybody can BS all the harder songs with HZD mode, since it's basically Light Mode that only allows 2 misses all the way.

Marketplace Rating: 0 | It seems like they patched it then. I'll have to check it out sometime later next week to do a confirmation. I might be wrong, but from my experience, that's not supposed to happen.
*edit*: Ok, I've checked the HZD status. It seems complicated. When there are no notes within the judgement area, and you pick, it is in Light Mode. However, if there is a note (including the one you have just picked) within the judgement area and you happen to produce an extra pick, it will be considered a MISS.

GF: 95% Perfects and above.
DM: 93% Perfects and above.
Usage of LIGHT or AUTO is not allowed as usual.
Requirements for Extra stage on V3 is also as usual.
