Yes, it is possible to unlock Carnival and Shin Hell 13. You can unlock them eventually.
You can unlock EX difficulty by choosing a song on EX difficulty during an Extra Stage.
Alternatively, when all songs and courses are unlocked, so should the EX charts.
How? A ps2 memory card cant go in a computer. (duh)
There are many options.
1. AR Max Evo (with thumbdrive)
2. X-Port (with USB cable)
3. Various homebrew software that can either use a thumbdrive or your PS2's network connection to transfer stuff to/from your PC

-Yes, they unlock via playing the various game modes and using the unlock minigame.
-Either get them as an extra stage or get all the other unlocks. If after unlocking Hell 13 a song doesn't have an EX, it simply doesn't have an EX at all.
-No. Use Challenge mode, which is just like the old Arcade Mode option, which I believe they've removed because it's redundant.

-Already answered, although in addition an AC song means it first showed up in the arcade, CS song means it was first in the home version. For example, Pretty Miyabi is a 12CS song, so if you find a 12AC cabinet don't be surprised when it isn't on there.
