To get the update to work, I downloaded and installed the full game as available on the konami website (the 469MB download) and then put the patch on top of that.
To get the game to install from the download, I had to set my computer's 'regional options' (in the control panel) to Japan and also made Japanese the 'Language for non-Unicode programs'. After a restart, the game and the update patch worked first time.
Now I'm not sure how much of that I actually needed to do but the language/region changes certainly made a difference after a long period of being stuck. Try applying the patch after changing your computer's settings before trying any of the other stuff I did.
To add the free songs, go to 'Add Install' within the game. From there, assuming you have an I-Revo ID, it's fairly straightforward pressing of the red button.
The things to watch out for when registering with I-Revo are the Kanji/hiragana name entry (which you can copy from the other thread) and the mobile email fields (which you can just make up).
