Author Name (only the original author can request hosting)
Link to theme, aim where you can be contacted, email, or PM
Host requested
So for example, If I have a stepmania theme, called FFV feat. SNES, i would post this:

Laser123454321.com, Stepmaniathings.com ( "All" for all applicable )

Site Author's Name

What you are willing to host
Preferred method of communication
