I usually recommend closing all unnecessary programs, to minimize the chances of random CPU usage spikes and disk accesses (will help greatly)
Does this happen on every BMS, or just certain songs?

It happens on every file, and regardless of what else is running in the background. I fresh rebooted several times trying to fix it and came up with no answer. and yes, it happens on every BMS.

sucky thing is that it's kinda tough to test if that's the problem on a laptop, you'd have to buy a decent laptop soundcard and that can easily be $50-$80.
i don't have any other suggestions at the moment, though.

Does it happen in both full-screen and windowed modes? On some computers I've had better synching in windowed mode.

and your V-Sync setting was forced...
the other problem was, maybe you using LCD...
so the screen seems un-synced well
